Relic altar not working

Hi guys,

I was busy last year and could not play Sanctum league at all, so I have no knowledge about it and just started playing it now.

I put in the Quest relic to start the run, but after I die, I cannot put a new relic in the slot


Can anyone help me with this?

You can only apply relics to new tome's. If you don't put a tome into the window, you can't apply relics.

After I failed, the tome disappeared and I could not get a new one from the Ossuary. Where can I find it again?
