Unable to re-enter forbidden sanctum

Hi,
I died in The Forbidden Sanctum and when I tried to get back into it I cannot. The Relic Alter has lost the Tome which I found in the felshire ruins to enter the sanctum. I have backtracked into the ruins and I cannot find another. What am i missing or is this a bug. Many thanks. Last bumped on Aug 30, 2023, 5:11:20 PM

Think I have the exact same issue, I've run a few sanctums, levelled a new character and now neither one can enter a new one. I can open the relic alter but dont have the tome to activate it?
