Rathpith Globe + Dissolution of the Flesh potential glitch?

Ever since I switch my Hexblast Mine build into the Rathpith + Dissolution HP stacking version I've been having this issue and it happens quite often. I'm not sure what's causing it and would like to know if anyone has a solution to this. It has ruined the League mechanic for me because I can't do anything after dying. All I could do is log out and log back in.
