Eldritch implicits

Hello there...
Some time ago, I found a list of the eldritch implicits, ordered by ember/ichor type. So I could easily see what kind of mods with what values the lesser can give, what the greater gives etc. But I can't find it again.
The list I found orders them by tiers and my problem is, that it mentions 6 tiers, even though the embers/ichors have only 4 stages (lesser, greater,grand,exceptional).
Can someone please tell me how exactly the 4 stages translate into 6 tiers?

The lower 4 tiers are 1-to-1 with the Embers/Ichors available as drops.

The two highest tiers, Exquisite and Perfect, can only be achieved by using Orbs of Conflict, which lower the tier of one modifier and raise the tier of the other. Exceptional can be raised to Exquisite, and Exquisite can be raised to Perfect.

[Edit] Also note that iirc "While a Unique is in your presence" can't be rolled on the lowest/Lesser tier, and "While a Pinnacle Boss is in your presence" can't be rolled on the lowest 2 tiers, Lesser/Greater.
