question about boneshatter helm enchant

the enchant that says "boneshatter has a 24% chance to grant +1 trauma" . does this apply two sets of self damage for the hit since im gaining two truama or does it only apply 1 self it, if lets say im taking 500 self damage normally and then the 24% happens do i take 500 again or 1000?
