Try this -->
1. Run packcheck.exe as "an administrator" / In steam select "Verify game files".
2. Delete the local PoE cache files here -->
C:\Users\"your windows account name"\AppData\Roaming\Path of Exile\
3. Now start PoE again as "an administrator". When at the login screen, select options.
4. Now set texture to MEDIUM and Sound channels to low.
5. Login to the game and remove ALL MTX from gems and gear (if you can't login to a char, then use another one and force all the MTX to be connected to this char and then try the other one again).
6. See if the issue is resolved ?

Depending if you use DX11, Vulcan or DX12 (beta) then try changing between them.

Also make sure that windows is fully updated including your vendor drivers.
Does this by any chance happen when you switch fullscreen to windowed, or tab out and back or such?

The swap_chain in your log is a Direct3D 12 call and some above related things are mentioned on MSDN.

If so, my advice would be to try and play PoE in windowed mode for a while and see if this still occurs. Just as a test.

If not, this post is probably moot.

Did you try turning it off and on again?
