+1 Maximum Fortification granted by Lethal Pride is not added to the bonus from Fortitude

So I noticed I had only 20 fortification, and before I had 22 (2 were granted by lethal pride "Commanded leadership over 11433 warriors under kaom. passives are conquered by the karui historic")

The Notable passive that give +1 maximum fortification with the above jewel are Inveterate and Quickstep.

It seems that 22 fortifications appears after I kill like 50 mobs, and dissapears when trinity goes off. Does timeless jewel fortification apply only in combat?

Also during map bosses fortification remains 20 instead of 22
Also some maps stay on 20 fortification (e.g. summit t15, al hezmin influenced)

Fortification in general only applies during combat. Stacks of Fortification have a duration of 5 seconds. The reason you're dropping to 20 out of combat is because of your Ascendency saying that you have 20 Fortification.

While I didn't look too hard, the only other source of fortification in your build that I'm seeing is your belt's "Melee hits which Stun Fortify". If you aren't doing large enough single hits to stun a boss, you won't get fortification from hitting it.
