Missing weapon, shield, and gems.

In Act 7, completed the optional detonation mission with the dude but died due to the explosions. Respawned outside the teleporter to Maligaro's map, teleported back into the map to kill the boss to find out my weapon and shield disappeared along with all total six gems that were in each item after I died! Don't know if it is a bug or what but they weren't where I died or respawned. I tried going to my stash and the crafting table they weren't there. I shut down the game and restarted it but both items and gems are still missing.

you properly pressed the weapon swap key.

However GGG doesn't offer any kind of service for tracking ingame currency and items.
GGG also doesn't offer any kind of ingame refunds or restores for items or currency like this.
Thanks HanSoloDK you are a lifesaver. It was a weapon swap key (Right trigger and right stick for controller) issue. I wasn't sure if it was a glitch or a bug as I hadn't seen any posts about weapon swaps before. It's a pretty cool game mechanic that I'll use going forward. Also yeah POE doesn't do refunds or credit or tracking I was just seeing if anyone knew what could have happened as I was panicking about where the weapon and shield could have gone who knew they were still in my inventory. Thanks again.
Update: Just finished another detonation event with the NPC Tujen from the Black Scythe Mercenaries. So the issue was that for the controller version you have to press both the left and right trigger to get to the detonation bomb option. While holding down both triggers you press down on the d-pad to see the green or red circle for the detonation blast. The reason for the weapon swap is that to move the detonation circle you move the right joystick. Doing this causes the right trigger and the right joystick to do a weapon swap automatically. (To put down the detonation bomb for the controller release both triggers and press the A button to put down the detonation.)

So if you're on controller you are going to do a weapon swap by putting down the detonation bomb. Hope it helps someone else.
