Fortify/Fortification stacks not working

Hi, got a strange bug or issue with fortification stacks.
On most boss, especialy those empowered by sentinel. I'm not gaining any stacks of fortification while attacking. I tried with fortify support and with the -3 fortify on hit masterie. Happened during leveling and maps. Even the X% attack dmg leeched as mana does not work with 1m dps. That's because of cyclone "smalls hits", but the threshold requeried is a way toohigh on new mobs, i can't get leech or fortify stacks on yellow map boss with 1m dps. Maybe the "fortification stack before mitigation isn't working" anymore, or the ailment threshold is too high ? Tried my best to be clear. Cheers and good luck.
