How are alters spawned?

Are they just random or are they based on the amount of monsters killed? It just seems that the alter spawn rate is so random. Some maps I get like 4 or 5 and some maps i get 1 or 2. Thanks

Sounds like you're crowding out altars with some other effect. Been a big topic with the new atlas passives. People are able to pump more stuff into an area than the areas were designed to hold. Have to be more choosy with what you pick until they come up with a fix for it... if they decide to try to fix it.
As far as I know, altars spawn based on how many "native" monsters you kill. Packsize is everything here, but added non-native monsters like Harbinger, Breach and so on have no effect. But certain mechanics you can add, does increase the amount of native monsters, as far as I know, like Domination. I may be wrong here, though.

It's RNG.

There is no magic number like killing 100 monsters = an alter.

It is based on RNG. For eg, every monster might have 1% chance to spawn an alter, just like everything else in the game.
What "alters" are we even talking about?
