Can't enter opened map portal, 'unexpected' disconnection

I was in a map at the boss, died, went to re-enter portal, and disconnected after a second or two in the loading screen. I cannot re-enter map, I get disconnected every time, though I have no problem spawning in Karui Shores and I can see other players running around. After a few times trying and getting kicked out of the game, my portals disappeared. Specifically, it was a t16 Kirac mission in Cold River if that is useful to know. Not sure what to screenshot so here's my login screen with the error message.

Similar issue to what just happened to me.
Inside the Kirac mission - disconnected out in the middle of the T11 Atoll. Logged back in about 30 seconds later, however all of my portals are gone.

Thought this was fixed seasons ago.
