Zana mission unable to complete because the Vaal Fragment never dropped

I have had this same issue previously. This time I made sure to screen shot it. I even opened all the chests I could find in the map.

I just had the same issue.

Unable to complete Zana Mission "Find the Vaal Fragment"

All monsters cleared.
Tried breaking all cairns and opening all chests.
Disabled item filter.

Had exact same issue. all monsters killed, no fragment.

Same thing here. Spent over 11 minutes running through the map like 7 times over shooting every square inch with contagion trying to find a barrel I'd missed.

Screenshot doesn't show it properly because I was rushing to try to get a screenshot with the bug report number, map info, Zana quest info (killing monsters extends time), as well as the timer. Even though I messed it up royally in a panic, there are zero monsters remaining.

Same problem with me, killed all the monsters, tried to open all chests.

Same issue but with a Map item.

I'm guessing that the monster with the vaal fragment is in the Scourge realm

Thanks for your report, we'll look into this.

I ran into this issue yesterday, and the item ended up dropping from one of the scourge monsters. It is possible that the game loads the zana quest item into one of the scourge monsters. If this is the case, I don't think it is a fair design, since not many players can keep up with the pace of full clearing the scourge-verse of the map.
