Inscribed Ultimatum did not drop the reward upon completion

So i bought an Inscribed Ultimatum where i have to offer a Skyforth and the reward would be a Maloneys mechanism, So i got that ultimatum and i went and bought some skyforth boots.

Double checked that everything was correct and slammed that into the map device, put the skyforth boots into the ultimatum window and ran the encounter, i completed it without dying and everything was great.

I waited in the middle as the loot explosion was happening but i never heard nor saw the Maloneys drop.
