Help - stuttering FPS / LAG Nvidia 1080 GTX 8GB

I have been playing POE for over 3k hours and through multiple leagues and have never been able to fix my FPS LAG stuttering issues.

I initially thought it was an internet issue due to me being on ADSL 2+, but I have since moved house to a new area with NBN which sees me having extremely low stable pings in other games.

However I still have issues when encountering large packs of mobs or casting large numbers of spells. This has persisted through multiple leagues and despite my best of efforts (see below for everything I have attempted) I have been unable to get steady consistent FPS.

I will list all of my settings and all of the guides, posts I have following/tried at the end of this post.

I am hoping one of you might be able to offer a solution that hasn't already been tried, or at the least get some use out of this post.

I have also attempted:
- editing the production_Config.ini to disabled sounds
- installing steam/non-steam versions
- running CCleaner and antivirus software
- checking my ping to the server using command line (no packet loss)
- clearing shader cache / temp folder (slight improvement in fps unstable)
- running winMTR no packet loss.

If you have anything else to suggest on how to fix these issues I am all ears.

**UPDATE** If anyone is interested. I ended up forking out for a 3070Ti on my old system. Unfortunately I still experienced stutter due to CPU bottlenecking. Turns out my 2016 system was to blame. I ended up upgrading to a new mobo and CPU and it now runs delerrium at steady 50fps. So if you want a surefire way to fix stutter this is the only solution I have found.
