Non working fps cap/incorrect fps readout

I'm currently playing on an M1 Mac mini with a dual monitor setup, when playing path of exile my fps counter says 120fps but when action happens and it says 86 fps it feels more like 30fps. none of the cap settings work and turning vsync on or off doesn't do anything. anyone else having incorrect fps counters?
