I have the same issue. Killed her a few times. The first time she died after I died due to bleed that I inflected on her. Did you both have the Maven actually witness the fight? While you have to disable the beacon to use Sacrifice fragments, you have to have it enabled while using Mortal fragments to invite the Maven and have her witness Atziri.
