Maven's storyline locked out

Hey,

My Maven's storyline is locked out due to the fact I didnt pick up a Maven's Beacon after the first encounter.

I've managed to kill the boss supported by Maven but I've died right after. The Mavens Orb has dropped on the ground but I couldnt go back to pick it up as it was the last remaining portal. Now my quest line with Maven is stuck as it wants me to use the Orb on the map device, but I obviously dont have the Orb.
In the Challenges section the challenge "Acquire The Maven's Beacon" is completed eventhough I've never really had it in my inventory.

Tried running the same map and the same Atlas area multiple times after that (25~) but she never appears again.
