Can't start last ritual

The last ritual encounter out of 3 in a t4 channel map doesn't have an icon and is not clickable. Other 2 in the map worked fine.

same exact thing happened to me on Channel. aggravating

Experienced the same bug in a T4 Ramparts map. Had three rituals and couldn't activate the last one. There was nothing to click on and the minimap icon for the Ritual shows that it's untouched.
Also, this is the first and only time it's happened for me so far. Reference #3,025,967,004  Avid door opening enjoyer.

Same bug in channel map, the Ritual is placed by the stairs, infront of boss area. Unable to click on the ritual, and the "circle" that marks the ritual area, is not in sync where the ritual is placed. It seems like the Ritual stem is supposed to be in the middle (where the boss spawns), but is placed wrong?
