No Access to Azurite Mine after death while delving (Synthesis League)

Tested twice and reproducible.

When dying in a delve and re-spawning, there will not be an entrance to return back into the mines. As a temporary solution, leaving and re-entering the mining encampment restores the entrance.

-Run begins from Mineshaft (Depth:15) to Azurite Cavity (Depth 16)
-Character level:55, witch, main skill: bane
