The Unbreathing Queen 2: prophecy doesn't spawn

I've run normal labyrinth several times. While previously this prophecy would immediately spawn in the very first room it doesn't at all now. I saw the problem reported on Unbearable Whispers 5 not spawning in lab too, so might be a more general bug
