Ruined Chamber not opening all of the time

When delving to a ruined chamber (vaal outpost) and triggering the door, it is failing to work all of the time. We have done 7 and the last 2 doors failed to open.

I have the same issue, out of 4 vaal outpost ruined chambers, only 2 opened. I have no idea if it's intended (but unexplained...) or bugged.

Me too, 5 Chambers found and only 2 opend.
