"unable to create index buffer" - Game or Driver?

I noticed this error has been reported a number of times, both here and in bug reports. As it's just occurred here for the first time, here's my thread.

What I can't decide is whether this is the game, Windows or the driver. Looking at the time stamps, the driver recovery is just before the game hang, which suggests the driver is at fault.

The Windows/Driver/Game configuration I'm using currently has been stable for few months now.
