Client crash: Buffer Underflow

I got client crashed many many time . The error say: Exception: Buffer Underflow. I play tri-herald bv elementalist, use 3 mtx herald. please fix this

I am running windripper rain of arrows with herald of ice, I have tried different MTX and they all crash the game.

It is always when I attack a pack.

I am running Burial Chambers.

.. i am confused ... play over 5 h ... i am only run Burial Champers ..... no bug or error ....
10 min ago the Buffer Underflow Error beginning ... i cannot run 1 map ....allways errors....
