Ashen Wood Boss Out of Map

Full cleared map. Boss is out of reach. She can hit me, but I can't hit her. All walls are blocked to reach her. Attached is screenshot and bug report number.

Update, I can hit her through a very small crack with Essence Drain. So whatever is generating the map accidentally blocks the path to get to her?
