Can't complete kitava's torments

I did the /passives to see which ones I was missing. Kitava's torments was one of them. When I go to the reliquary there is a green container I can click, but when I do nothing pops out.
I've now cleared the reliquary 3 times to see if it would change and it did not. Is the /passives bugged or is the quest bugged? I can't remember if I completed it or not before, but it doesn't show up in the completed quest section either. Last bumped on Dec 27, 2017, 11:20:08 PM

There are three containers with three relics. Maybe you already collected and delivered one but not the others? The one you already got would not drop again. Check if you can find the other two.

I have completely mapped the entire place 3 times and there is only one container I can click. (which has nothing in it)

Are you sure you're in the correct act? This quest is done in Act 5. Maybe you're checking in Act 10?
