Unbreathing Queen V didnt drop grip of the council

Just ran Unbreathing Queen V. Got the key, but no grip of the council drop. Whats going on - is the drop not guaranteed?

Same thing happened to me. I'd also like to know whether or not it's a guaranteed drop.

Same thing here but with Plaguemaw prophecy not droping Breath of the Council. If the drop is not guaranteed then shit is too hard to get, makes no sence. Completing the 5 prophecys is already tied to rng for activating them, getting a second rng check makes no sence with the actual state of silver coins drop rate.

It is working as intended AFAIK. The Fragment drop is guaranteed. The unique corresponding to each council member's chain has a CHANCE to drop when you fight them in the final piece of their chain.
