+1 Chaos Gems recipe

Hello all. I feel like this is going to be the dumbest question ever, but for the life of me I cannot make the +1 Chaos Gems recipe to work.

I'm following the wiki Vendor Recipes page:something:



I tried with and without the Orb of Alteration. The gem is a Blight gem.


I'm preeetty sure that I'm missing something and I will feel like a complete idiot when I realize what it is.

Any hits? Thanks!

Don't you also need a ring?
