Prophecy "Ancient Rivalries IV" activated but didn't complete

As the title says, the prophecy triggered and i was wearing Victario's Charity in my second weapons slot but it wasn't completed and it still used the prophecy up. I just bought it for 25c too :/

I don't have a screen shot because I thought it was completed and when i went back to the Harvest the prophecy was completely gone.

Did I have to wear it on the main weapon slot or something?

If it was in your weapon swap, which is what I think you mean, then you are correct. You do have to wear it in your main offhand. As long as it is showing in your inventory (and equipped) when, in this case, Malachai dies, you are good. Having it in your weapon swap and not active doesn't count, as you just found out. :/

"

The unique items you have to wield or wear to complete these prophecies must not be corrupted. For weapons, placing the unique in the alternate weapon slot is not sufficient; it must be actively equipped by your character. Sockets of the unique item are unchanged.

While this quote is for fated uniques, this also applies to any item that has to be equipped.

Ancient rivalrlies does not get consumed when you don't complete it. Check your prophecies list again!

There are several commonly mistaken prophecies that will appear on screen but not be consumed when you create an instance,

- Twice Enchanted (the prophecy is only consumed when izaro is dead)
- Ancient Rivalries (I know for a fact because I died in breach HC to malachai stupidly while doing this quest, then i finished it on standard. Because the move to standard is new league, I had to create a new instance and could not join my previous instance of course)

- From the Void (will appear in your 'prophecy quest log' to the right in any beyond zone
- Fated unique prophecies items as stated above will show when you got the foe in the instance

There are probably way more but I don't know for sure.

Make a new instance and do malachai again!

If you can confirm it is gone from your prophecy list then we got a bug I guess, or perhaps they changed how it works.
