No Stone Unturned - Can't Get Achievement

I have clicked every lore, but I haven't received my achievement.

Last time I checked, it said i was on 8/9 for Act 2, but i have visited:

Crossroads - 4 scrapings on wood (in those deteriorated houses)
Chamber of Sins Lvl1 - Plaque on the device in the central round chamber
Chamber of Sins lvl2 - Transmuted Device at the endboss
Fellshrine Ruins - Monument at the entrance of the big ruins
Western Forest - Letter in the Camp close to the blocked road to act1
Western Forest - Monument inside a circle of stones



EDIT: I justed clicked a random lore in Act 3, and got the achievement, so all good

Saw this post and figured I post here rather than opening a new report.
Just got the last "Other" item for No Stone Unturned Achievement, but was not awarded it despite it showing I have read all the required items. I tried clicking on past lore to see if it will trigger it, but no success:
