instant death for no reason

I was busy manageing my gem inventory in a sinkhole, removed one of my gems(it was added lightning connected to firestorm), then all of a suddern, with no monsters dealing damage or anything, my health bar hit zero, my energy shield was unnaffected, I have no idea what caused this bug.

That shows you died to chaos damage. It bypasses energy shield. What kind of monsters were in the corrupted area? What mods were in It?

Desecrated ground?

it was the earliest sinkhole in the game(no notable modifies except lots of totems), single hit with no enemies even next to me or anything(as in there was nothing TO attack me, this wasn't even at the point where chaos is a thing yet), the closest enemy was a totem that was completly out of attack or aggro range.
