Have you tried editing the ini file to enable fullscreen before starting the game?

Post a dxdiag in Spoilers please.

Maximum character count exceeded, use pastebin instead:

This is the log from my new system installed 3 days ago. Either Win server 2012 R2 or 2008 R2 (previous one) has the same problem.

Additionally another bug in fullscreen windowed mode:

Whenever monitor power goes off, either manually with power button or automatically by energy saving policy, the game window will be stretched a bit larger after monitor power on. In order to restore I have to change the screen mode to windowed, and then back to fullscreen windowed.

That sounds like there might be a cable issue, do you have any other DP1.2 cables to try?

It looks like you can get the monitor driver here. That might be better than the generic windows driver.

Thank you for you information, but I don't think this could be cable issue. The monitor works good in 3840x2160@60hz fullscreen windowed mode, which shall be the same effort as real fullscreen mode.

Neither could be driver issue as well, I just upgraded monitor driver and it doesn't even interrupt onscreen display, that indicates the monitor works in the same mode b4/after driver installed.

The newly released nvidia driver solved this problem....

The video driver fixed this?

No, just the fullscreen problem.
