Vaal Oversoul killed, quest not completed

Hello,
last night, I fought the Act 2 boss, I died once, and hence casted town portal in the room. Then I was able to kill the boss, I picked up some loot and went through the new opening. Then, I checked my inventory, but somehow, the items were gone. I activated the waypoint to see if I could get back to the boss fight room via town portal, but that was gone as well. Things got weirder, when I tried to get back to Act 3 via waypoint, the waypoint said "You do not have the prerequired quest state", i.e. I didn't complete the quest required, although I had just killed Vaal Oversoul. Below, parts of the client.txt from the PoE folder, not sure if that helps, but it's all I can offer.

I played with a Duelist, using mostly Heavy Strike, though I doubt, that this will be of use here. I certainly picked up some items on the floor and they were in my inventory, yes, I checked that. Same here, except I didn't lose the items. After I first killed him I took some loot and went to the forest camp( didn't go through the exit), spoke with everybody there and the quest wasn't complete but my portal was gone. I had to start back at the last waypoint, fight my way to the Vaal Oversoul and kill him again ( I was able to activate the altar again even without the apex). Not really a big issue - I had a ranger and vaal oversoul was quite easy once you learn its attack patterns - but still...
