How does elemental equilibrium when you deal ice/fire in a single attack?

If you're using a support gem like Cold to Fire, how does this work with the keystone Elemental Equilibrium? The keystone gives the monster 25% to the resistances you hit with and -50% to others. So if you're simultaneously hitting with Cold/Fire damage, would the monster get -25% resist overall to both? Does this keystone even work with support gems or would it only take the element of the actual spell into consideration? Does it work with support gems that add a flat amount of damage like "Added Fire Damage"? An explanation would be appreciated if anyone knows.
I believe the current behavior means that if you hit with both fire and cold at the same time, the monster would get:

+25% cold resist
+25% fire resist
-50% lightning resist


If you hit with only cold it would be

+25% cold resist
-50% fire resist
-50% lightning resist
You hit with fire and cold, so the monster hit gets +25% resistance to those two elements. Doesn't matter where they came from, only that you dealt those damage types in that hit.
You did not hit with lightning, so it gets -50% resistance to that.

Also yes it considers all the damage that you deal with an attack or skill, regardless of if it comes from the base damage of a skill, a support gem, or added damage on gear.

How long does the effect last? I am guessing that it is until the next attack inflicted on the enemy but I cant be sure.

Either 5 seconds or until the next time EE would affect that enemy, whichever happens first
