stuck tavakal fight between phases

Hi,

I was fighting Tavakal the Fallen in act 4 and my character got stuck inside the house after Tavakal leaves outside when the 2nd phase beggins. I can't use any teleport, I can't get my character killed to be taken to the nearest checkpoint. I have tried to restart the game, going to character screen, trying to log in with other characters and then log in again with this character and still it's stuck.

Character name is Donmonkgo.
