Lord of Ogham Alternate Back Attachment

Does anyone know how to enable this?

I bought the support pack but can't seem to find a way to enable and select the alternate "Corrupted" version where it looks like tentacles.

Tried scouring posts but couldn't find anything that works. There was one post on reddit about seeing roman numerals in the attachment slot but I don't have that.

I'm on PS5.
