Shaper's memory fragment spawned when it shouldn't have

I just ran a T12 map with shaper influence, and upon killing the boss of the map it dropped a Memory Fragment IV. There are several reasons why this shouldn't have happened:

1) It was shaper influenced, not elder
2) I have already completed the quest that drops a T10 shaper orb fragment from a map boss kill
3) The map was too low level to drop fragment 14

As best I can tell, the bug occurred because I spawned elder before I reached the boss of the map, and the only elder guardian I have not yet killed spawned on the map I was running causing the game to somehow think I was killing the elder guardian instead of the normal map boss.

As a side note, turning in the fragment to Zana did yield a T10 shaper orb, but did not progress the quest line any further (it still says I'm on 13/15 fragments).
