Blood Altar unique jewel recipe didnt work

Hi, i just tried to create unique jewel from bestiary recipe in blood altar, but after killing monsters (and not dying)there was no jewel in the altar! Previous recipe for unique item worked all right. Please investigate my account, this just happened 15 minutes ago. I hope you can credit random unique jewel or some random mtx to my account for this big loss :)
