The only cosmetic effects you can get without spending money are the challenge league rewards.
Right now, for about three more days from now, you can get a free mystery box when you spend any amount of points - but you have to pay for these points. The mystery box contains one of a wide variety of cosmetic effects. Finally, every once in a while GGG runs various promotions which can reward free cosmetics.
