Items dropped by mercenaries disappearing

This has happened to me twice now.

I challenge a mercenary for an item, and win, I see the item drop and hear it, then it simply isn't there any more about half a second later.

This isn't a loot filter issue, I have tried disabling it, besides which the items in question would definitely show up on my filter.

Quite frustrating really coz they have both been really nice items.
