Missing items in Ritual

This is not the first map where I do the same thing, I kill monsters start a ritual, finish it without checking the rewards and go to do another one, and it happens that after the last ritual the items are missing in the first slots, I don't take anything out of there, I play alone, it happens on different maps,when i reroll all looks fine, I don't use scarabs, I have taken everything pertaining to the ritual in the atlas tree, surprisingly if I'm not mistaken I already had the same arrangement meaning that there were missing 5 let's call it slots(empty squares) with no rewards I don't remember if it was identical in each one but for sure once it was identical as it is now. I don't know what other information might be useful.

I thought the same thing because it happened to change the character but it happens until today, for example today I played all the time on 1 character, to that I did not leave the map, I did not get kicked out of the game, etc I do the map and a few rewards are missing in different places, for a couple of rituals it happened today with 2 times, and I act as I always that is as I wrote before, I do all of them and at the end I look at the rewards etc.
