Cruelty Support's effect increase is not present.

Cruelty support gains increased effect per level and per quality but, using the game tooltip after hitting something (Worm flask for consistency) you only gain exactly 50% more damage instead of 62.5% for 21/23. It doesn't even work with a 1/0 gem, as it has by default 10% increased effect but the tooltip dps still shows a 50% increase instead of a 55%.

Seconded, just tested it, too: Hit mobs on ledge with cruelty 1/0 and 20/20 and they both showed the same tooltip dps after gaining the buff - Max effect is fixed at 50% more dot dps, no matter the gem lvl/quality. Should be 62.5% at 20/20 and 55% at 1/0.

P.S.: Maybe it would help, if the description of the cruelty buff was less cryptic, too. Aside from the buff values for 0.01% of HP/hit and 3.2% of HP/hit, we have no way of telling how good the support actually is.
Confirming, this stat is broken, and extra levels/quality have no effect on the gem.

Thanks for your report, this is intended. Cruelty will only supply up to 50% more damage over time, the increased effect only makes it easier to hit the 50%.

This is similar to Shock where no matter how much "increased effect of Shock" you have, you will still be limited to 50% increased damage taken (except in special cases such as when using Voltaxic Rift).
