Blade Vortex + Harbinger crashing game

On my Blade Vortex Occlutist, any time I Dash into a pack with Blade Vortex already going, my game crashes. Roughly 40-50% of the time, the instance also crashes.

Have similar issues with flicker strike. When I teleport into large pack game freezes completely for 10-15 seconds which is lethal in most cases.
