Help with carrion golem build

Hello,

I'm currency playing carrion golems and I feel like I got a decent gear but I cant seem to do maven big invites. Like 4 breach lords and stuff due to my minions dying. I also got only 1.5 mil dps even though I feel like at this point it should be around 3 mil already. Could someone take a look at tell me what's wrong?
