The first one is the regular skill gem, the second is the corrupted version which is called a Vaal Skill Gem.

A Vaal Skill is simply a additional skill that fills up with souls till it's fully charged from slayed monsters.

So you "can't" usually use them all the time, only with certain builds that build around that.

Vaal skills are situational skills that you only use when you "really" need it.
Be it damage, defense or utility.

¨Vaal blade vortex is OP as fawk it basically more than doubles your damage when you activate the vaal.
