Cadiro Perandus does not spawn (PC Steam version)

I play in SSF Softcore Harbinger League and run perandus mod on shaped maps (t11-14) + colosseum map. There is "Has 33% chance to spawn Cadiro Perandus" string in map description, but Cadiro doesn't spawn within something like 20-25 maps, only chests with guards.

I did 25 T14, 25 T15, 16 T16 in past 3 days. Got 2, 1, 0 Cadiro retrospectively.

But probability of no Cadiro is 0,03%-0,004% (within 20-25 maps), it's nearly impossible. So, if it really possible, GGG should do pseudo-random, because it's absolutly disaster for any SSF/poor player: so high possibility (and cost) give so little good outcome
