Disconnection Headache

I have been getting DC's pretty hard since i started playing again , and by pretty hard i mean i get into any town, 10 secs later DC. And i would like some help to identify my problem , So here is my winmtr results:

The game ports are open in my firewall if any one is wondering.

Hope some one can help me out.

I think its the 5th time i experience this after the path of exile patch, everytime i go to other map this happened.
