One stack of Wither is one "skill use" and I doubt each cast costs 100 in your build. Also, having another trigger in Kitava's might interfere? Not sure.

Any news on this? Mashing Flameblast 10 times a second is wearing out both my mouse and hand :(

Your bug report is confusing. Are you trying to proc Kitava's thirst by casting wither, or are you trying to have wither cast by Kitava's thirst?
Also, cwdt in Kitava's thirst won't work - each spell can only have one trigger thing supporting it. EDIT: Ooops, didn't see it had been bumped. Still the OP's report is confusing. I believe OP has the samme issue i am facing, thus the bump.
All Wither, Incinerate and Flameblast trigger Kitava's Thirst only on the initial cast, but not when spending Mana to channel. Since it is indeed possible to get a high proc count by furiously clicking, instead of channeling, it appears like a design inconsistency/bug, that it is not possible to just channel...
