" A mechanical and visual bug. The maker of rain bosstype can still have her rain of spikes going on after her death. In this case the mechanical bug was that it kept going for so long. The visual bug was that it didn't appear on screen(same way many moves such as piety's ball lightning sometimes don't appear). So a pretty shitty way to die. I love the desync shown there too. (the one shown just before he dies)

It would be fascinating, and educational, if we could see a side-by-side video of what was actually happening on the server. If there's a way GGG could stage a demonstration like that, it would go a long way towards clearing up myths about desync.
