Low FPS / Psyeduo Lagging / Being Kicked to Login Screen

I stream games, However while encountering this i am unable to stream.. I've tried with the Stream software (XSplit) and without.. Neither helping the FPS issue.

I've attempted to Lower the game settings all to no avail.

I run a Alienware Gaming Laptop (Here's Specs)


The Laptop does have integrated GPU... I have went to the Nvidia Settings already and Force Changed it to use the default (Nvidia 555m) as the default 3d GFX card... Nvidia also for some reason recognizes Path of Exile as "Phantasy Star Online 2"

The settings on this cannot be changed.

I have Right clicked the PoE Shortcut and clicked "Run with Nvidia Geforce 555M" Gfx card.. and nothing helped.

I'm at a loss for words.. it just feels so choppy.. here's a screenshot of the ingame FPS grid



It caused me to actually quit earlier today and give up playing for a few hours because it infuriated me.. I love playing to compete on ladder, and streaming while doing so, and this has completely stopped me from that.
