When playing for witch, with passive Chaos Innaculation(1 hp), in location waterfall caves 1 and 2, smthing kills me instantly, and then i see a message like i Commited a suicide, may be i am noob and don't know wtf is that, but i think its a bug

Check your flasks. You probably used a caustic mana flask or whatever.
